Form 637, dated 10 December 1964 and attested five days later: the last of the safehouse papers here.
The holder must use the property under the Owner's directions and relinquish it on request. That obligation binds his estate as well. The form separates the individual who holds the title of record from the party whose official business the property serves.
The memoranda's own phrase for the work, in the renewal of 30 January 1961.
The status of Subproject 42 on 5 September 1963, six weeks after the recommendation that the testing stop.
The status sheet's cost matches the annual sum in the inspection (Report of Inspection of MKULTRA — 2019 release package — source PDF page 17). Together the two documents connect the budget figure to the continuing field-testing facility, without requiring a reconstructed total from the surviving invoices.
January 1955, the third number for the same arrangement. The 1976 sheet says the facilities moved to San Francisco.
The cover description of the man running the premises. The 1976 briefing book quotes the phrase back, beside his name.
The Agency's statement of what the facility was for, in one sentence, over Gottlieb's signature.
The waiver concerns receipts from the people administering materials, not every record of payment. Hall's withdrawals would still appear in his account, and each operation was to be reported to the Chemical Division chief and approved with the monthly accounting. The parallel copy names Hall and San Francisco (MKULTRA briefing book — source PDF page 126).
June 1956, the second extension, in the folder's standard formula for why the premises stayed open.
The agreement is with the man running the premises. It is not in the folder and no leaf here quotes it.
Twenty-one December 1955, certified over the signature of the Chief of the Chemical Division. The address is withheld.
The recorder and four microphones belong to an inventory that also lists bedding and furniture. Read beside the audit's description of monitoring spaces and experimental photographic surveillance (Project MKULTRA, Subproject 42 (with attachments) — source PDF page 206), the list shows observation equipment embedded in the apartment's ordinary household contents.
The approval is dated after the project's expiry and extends authority from that earlier date. It needs no new funds. 'The progress of the project' is the entire explanation supplied here for another six months of operation.
An Agency auditor had gone through the safehouse accounts. What follows over four leaves is the answer to his report.
A private account could hold Agency money while a separate document recognized the Agency's interest. The declaration at Project MKULTRA, Subproject 42 (with attachments) — source PDF page 1 shows the obligation extending to the holder's executors and administrators, so control of the property would not end at his death.
The auditor's query brings the surveillance arrangement into view: openings between apartments, a bedroom window for observation, photographic equipment and repairs intended to hide the operation. The payments also bought cooperation over permanent alterations and release from the lease. The final instruction is to itemize such expenses.
A $25 premium on a shipment of household effects. The certificate is in the folder and is not reproduced here.
The answer says activity reports were generally filed separately from the MKULTRA files. That is an explanation of this auditor's missing reports, not a claim that none were written. In 1963 the inspectors again found incomplete files and heavy dependence on officers' memories (Report of Inspection of MKULTRA — 2019 release package — source PDF page 29).
The Chemical Division and the Staff above it. One signature is on the paper; the other is under a box.
